Creating performance management system on data in excel

We would like to develop a PMS system that would track performance of every employee and also create/initiate notifications for promotion. Every employee is measured on two criteria - Performance Report (PR) & Qualitative Assessment (QA) Report. Both these are tracked on a monthly basis. Below is the sample data for PR & attached is the document for QA parameters. Promotion criteria - One has to meet PR triggers to be eligible followed by a cut off score of 3.5 in QA PR Triggers are as follows - 9 Months - 100% 12 Months - 90% 15 Months - 65% 18 Months - 50%
