Project scaffolding for very simple ASP.NET application

We are creating a small application that is forwarding users from one system to another.
This project is used to get started and create the basic implementation including a database structure with just two tables, a small controller code and basically on web form.
Basically the user clicks on a link, is authenticated with Active Directory, some calls are made to external services and then he is forwarded to another URL.
The implementations for the two external systems will be done by us after this project. A dummy implementation is provided in the attachements.
Please find attached a detailed concept for this simple app.
Your estimate should include the scaffolding/initial implementation of the described components, the installation on our development environment and an hour of coaching (teamviewer).
Additionally we might need help installing it in the production system include another hour of coaching via teamviewer for that.
The code needs to be executable in the described environment (see attachement.)
All source code must be provided to us.
