Script a survey

I need a survey scripted. The goal of the survey is to collect answers to questions of a legal nature which will then be sent to and analysed on my own application.
However, I'd also need feedback on what would be the best survey software.
A number of features of the survey:
- the same questions will be sent to many different people within one company, each of them answering from their experience in their specific role/responsability
- most answers are defined in a list, but there should be the possibility to add answers that are not in the list (through eg an "other" function). Note that each added answer should stand by itself, so it should be possible to add various "other" answers. This is important as my applicant needs to display each separate answer individually.
- answers from all these people to a specific question must be collected and presented as a list in a new survey for another person.
