Cleanup Enrollment Form Page

The Enrollment form page must work well on all devices from tiny mobile to very wide screens. The solution is already nearly complete, it just needs to look pretty on all devices and match the style of the rest of the site. Freelance developers will not be granted access to server backend, but will be able to view and download the existing working pages. The solution should be delivered as code snippets (HTML files, CSS, graphics, descriptions, etc). Ideally, the entire delivered solution would consist of separate demonstration files (HTML, CSS, image, js, etc). Please read the attached PDF, estimate the number of hours, and offer a fixed price.
