Extract data from an online database - web scraping

I would like to extract all the data from this online database: [login to view URL] The data is held in the following 22 fields, which are contained in the "List of reports" tab once a search has been requested: 1. Report number 2. Report date 3. Trade name 4. Sponsor 5. Manufacturer 6. ARTG number 7. GMDN term 8. Device classification 9. Sterile 10. Single use 11. Model number 12. Software version 13. Event description 14. Reported event outcome 15. Report source category 16. Event type 17. Other medical devices reported as being used [Either “NONE” or:] 18. [Sub-field of Other medical devices reported as being used] Trade Name 19. [Sub-field of Other medical devices reported as being used] Sponsor 20. [Sub-field of Other medical devices reported as being used] Manufacturer 21. [Sub-field of Other medical devices reported as being used] ARTG Number 22. [Sub-field of Other medical devices reported as being used] GMDN Term I estimate that there are around 45,000 records, so circa 45,000*22 fields to extract. I would require the data be in CSV format, under the column headings of the field names.
