Script to ask user for input and update CSV files and do one transformation

We need a script that a user can run on a Windows machine that will ask them 2 questions.
1. What is the name of the files you are importing (with 5 different choices)
2. What date is the revenue data from (this will be a calendar input)
The selection from option one will add a new column to 5 CSV files in a folder and the rows populated with an Id based on the selection from question 1
One CSV file will be populated with the date selected in #2
One CSV file will be transposed as per the example CSV file
