Hi!, we can do this as a Windows App, with a list of checkboxes, so you can go around with a Tablet PC and tap. We can save the data in Microsoft Access or Microsoft SQL Server (Recommended) and also we can export it to Excel with a click, so you will get the Excel file with all the data to do the reports. charts etc, We have done this many times, if you have any doubt we can do a proof of concept for you without being awarded, to take out your uncertainty and demostrate with facts our words. Hello, As a person who enjoys using excel and creating useful sheets that make life easier, this project jumped out at me. I had a look at the form you attached and I have some initial thoughts and ideas. My inital thoughts/ideas are to make the form you fill out a digital excel file where you can input the data (or have someone input the data for you), then link the data to some formulas which constantly update certain stats or names of people who need to be observed again. Additionally, organizing the data well with headings will allow you to query, sort and filter as you need. You'd also be able to create graphs and tables if you needed them for any reporting.
