design mock up and graphic user interface for tablet mobile application

I would like to design graphic interface for a mobile application - the application run on tablet Samsung galaxy Tab S2. The application will be used like this:
1. User run application
2. There are a button & 4 textbox
- The button help user to take picture (the picture will show on the screen) then the button "Enter" appeared
- if user have no shoot picture, they can fill information to text box: there are 4 text box

The press "Enter" button
2. After entered: application show quizs, user answer the quizs for scored. There are two kinds of quiz: single select or opening answer(input to textbox). One kind for each question.
3. The end of quizs (user can move back to previous question), user will press the button "Finish"
