Need Website Designing & Building Will Contain Little Animation

I need a Website, Logo, Letter head, and (Report Form Template to work on a tablet)
My work will be property inspections looking for defects, this work is refereed to as Snagging.
However, I am looking for a unique design, with a little animation mainly for the landing page!
I have ideas to animate the navigation system, and page turn overs, don't like standard static navigation buttons prefer images to represent the type of page it relates to.
The Report Form template, will have a cover page with fixed pre-written section and logo, then it will have 4 sections that require individually filling in on site.
The report pages will be a series of 6 columns across, with rows approx 5mm down the total length of form (A4 size) this creates sort of boxes, 2 of those boxes on right side need programing to operate with a tablet pen so when touched with the pen one column box displays a green tick the other column box displays a red X the red X box then expands turns red allowing inspector to type the defect found in that box.
Sounds worst than it is, hope there is the right person out there without silly prices.
