One Page PDF Price List Design and Layout + Logo creation - URGENT -- 2

Hello there, I need to get a one page document designed urgently. I can provide the content but I need someone who is good at design to put it together looking like its from a corporate company. I will give an indication into the style but am open to suggestion too. I want something like a "pricing menu". I have montly pages at four levels so would like these displayed proffesionally, I want this price list to make my company look serious, corporate , simply laid out and stylish. I will rehire the same person to provide me with duplicate graphics style on my website so there will be ongoing work. Budget $40-$50 I also have three logos I like, i had one doe but have changed my mind so I need a new one doing too please. Please bid low in the view of the ongoing work, this is a quick job I just dont have the skills or the software to do it.
