A) My ministry has branches in different places and I want each branch to be able to send in their monthly reports and testimonies online. All they need to do is to fill out the report form and send. The two forms concerned have been designed (please see attachment). I want these forms to be put on our website just as designed (or with some improved amendments) to satisfy this purpose.

The sender should also receive an acknowledgement email (see below the forms) after they have sent a completed form.
All spaces on the form must be completed before the form can be submitted.
On the monthly report form, numbers 5 and 6 should allow as much information as the sender wishes to include. The boxes should be a text-area. The same thing goes for the text box in the second form (for Testimonies).
To prevent spammers from using the form, a simple captcha code should be included. The captcha code should not be those complicated ones that are difficult to decipher.
B) Secondly, I need amendment to the date feature on our online Daily Devotional Guide on this page
You will notice on this page that there is the Reading Date, there is Date Picker to the right of it as well as Today’s Reading. Both the Reading Date and Today’s Reading should correspond together. Right now, if you use the Date Picker to choose a date, the passage will change correctly to the content of the date chosen but the reading date will not change to correspond with the passage. Instead, the Reading Date will show Today’s date.
For instance if you click on the Date picker and choose 10th of July, the passage for that day will be displayed correctly to show Zephaniah 2:1-15. But the reading date will not change to 10th of July (which is supposed to be the case). I want this correction made so that whatever date is chosen will be correctly displayed as the Reading Date. The correct passage for that day should also be displayed.
The calendar on the home page also acts as Date picker and should correspond as explained above.
