Template for a Game Salad Quiz

I am designing a quiz app and will create all graphics. I would like a template created that can do the following:
Part 1: Quiz 1
There will be 25 questions (one on each scene).
On each scene there are 2 answers : One for a Answer A and one for Answer B
I would like a template created so that it calculates what the person picks and shows a % of chance of Answer A and Answer B on the final scene.
Calculations can be posted to Facebook or twitter.
Part 2: Quiz 2
This quiz has 10 scenes and questions. On each scene the participant can put in data. At the bottom of the page the participant can determine if the data placed in is correct, incorrect or close. On the final page the score is calculated and can be posted to Facebook or twitter.
