Simple Javascript form/functions with XML input

This small project involves creating six diferent types of forms that use JavaScript functions to process questions in multiple choice, true/false, matching, interactive table format. The input must stored in XML files (structure provided) and processed through XSLT for display on a Web page. Your experince and the quality of your deliverables is more important than being the lowest bid for this project. The details are outlined in the document attached to this post. YOU MUST HAVE experience in JavaScript, XML, and XSLT for this project and you must provide examples of similar work. You must also summarize your approach for this work. Generic bids will be ignored. Your success with this project will likely result in addtional work.
