You need to configure [login to view URL] OR Jquery file uploader OR [login to view URL] ( FOR YOUR CHOISE, ANY YOU CAN HANDLE) to this 2 file input fields. Uploading should start on pressing submit button.
Everything else is already configured ( AWS S3 bucket etc..)

Hi there, Looks like an easy task. The main question where do you want to upload the files? Heroku has their own file system that can not be used for file storage because they will disappear after dyno restart.
