I don't think it has anything to do with Apache SSI Module. SSI integration is pretty straightforward. I'm pretty sure its either the PHP version or missing PHP Modules. Did you checked the apache error log?

Hi! I've got 20 years experience with Apache and over 10 years experience on the AWS platform. I can tell you up front that if you are running on a Linux EC2 instance or inside their container service (ECS/EKS), there is absolutely no reason that Server Side Includes (mod_ssi) shouldn't run - you may just need a better SysAdmin to help set it up.
