ms word to ms access report

i have a work contract in ms word that i wanna put it into a existing ms access database as a report.
so data will come from a ms access table and fill in the report that will be printed for signature.
where is TABLEFIELD means data will be feeded from a table, so just put any name in the collumns and i will replace it with my table.
