HTML5 Mobile BarCode Scanner

Hi,

I am looking for a simple HTML5 Mobile App which has a textbox and small button next to it.

User can type in data in that textbox. When the user click the button next to it, it should access camara and scan the barcode (code39) and display the value in the textbox.

It should be great if there is no browser restriction.
It should work fine in all mobile devices.
