I need a small online database for a cemetery. The data to be stored consists of: • Last Name of Interred person • First Name of Interred person • Middle Name of Interred person • Date of Interment ??" year only • Block • Section • Lot • Purchaser Last Name • Purchaser First Name • Date of Purchase ??" year only • Notes I need a public view page which will show ALL the records alphabetically by Last Name. The fields that can be viewed on this page are limited to: • Last Name of Interred person • First Name of Interred person • Middle Name of Interred person • Date of Interment • Block • Section • Lot If there is no date of interment the record will not exhibit on the public view page. I also need an admin section which is password protected. In the admin section I need: • A page to add records into the database • The ability to search and display records at least by Block and Last Name fields • An edit page which will show all records with edit/delete options • Change password option • Log out I need the functioning MySQL and PHP pages loaded only. I will make them “pretty?? later. Of course, your design and layout should be clean.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
