Custom Icon & Buttons

We are looking for a custom icon for our Video Assistant Application. I have attached our video reel that we want as the icon. I have also attached an .exe file with all the buttons that we want replaced with "XP" style buttons. Last but not least, we also want a custom "splash screen" designed that matches the rest of the application. Please provide previous work that you have done in this area. A few samples are appreciated.
