Very Easy and Quick HTML/PHP job!

Hello, I need a website someone that I can trust for a job which includes, 1 - I need you to take a pre-made HTML code and simply insert it into a design on my website (custom fit). 2 - On a certain page I keep getting a error (error on a certain line in the code) at the top of the page I need this to be eliminated. 3 - I need you to install phbb3 script on my website and put it into an iframe to make it fit into the design of website. (Again custom fit to design). 4 - Put Google analytics code on all page + another similar to Google Analytics code on all pages of my website as well (approx. 10 pages) 5 - After a customer pays I will provide you with a special code which asks for customers physical address + telephone # ... I need you to code something special to make it obligatory for them to fill out all information before continuing.... REQUIREMENTS: (If you can't respect this, don't post here!) - I prefer communicating via Gmail, Google talk & Skype. Video chat is sometime necessary to ease complex explanations. - Turn around on replies is 5-10 hours max, if you reply after 24 hours, it's not going to work out. - You need to be able to speak and write in english, if you cannot explain to me something in email or via chat forget it. - I need a hands on guy/or team to make sure what I say gets done in the technical side of things quickly, and I can be updated all the time on the project. If you are the kind that takes hours to insert a piece of code to the website, don't even bother posting. (I know how to do most of this stuff and know exactly how much it could take approximately). - If you need to ask me questions every single time you do something, and cannot manage or research on Google how to do basic things on your own, do not post here. - If you are limited in knowledge with managing website and I'm talking about design (photoshop skills) all the way to basic programming of HTML, java script, flash, do not post here. - You need to be respectful and trustworthy, you also need to be a cool guy or girl ;)! My requirements are not evil at all, I demand that the jobs get done quickly that way you have more free time to do what you want and the same for me. I am not going to spend more then 30$-50$ max on this before I know how to do this myself but I simply don't have the time to do it, if you are experienced you can easily finish all this in less than 3-4 hours of work.
