WEB APP (jQuery Mobile): Dental Surveys - Form Inputs, Database, Retrieval

Introduction: I am looking at working with a new back-end developer to compliment my team (on a per-contract basis), who has the capacity to work full-time on time-sensitive projects like this. Our web applications are developed on the jQuery Mobile platform (to help cross-device compatibility), so a good level of familiarity with this is essential! If everything I request is met, and you're as easy to work with as I am, then I will have many more projects to offer you in the future! Important Points: - The Front-End and full Functional Specifications document will be finished by January 15th latest. I go into obsessive detail within the Funct Spec doc, as well as commented front-end code; you'll thank me :) - I am looking for a maximum << 2-week turnaround >> for the back-end, testing/troubleshooting included. I will be on-hand to assist with the latter. - Communication and English skills are extremely important to me. Frequent updates on progress is required to feed & assure my client. - You may use your own server for development, and upload to mine when a milestone has been reached and rendered. Context: The {removed} provides informative in-class workshops to students and adults on the importance and correct practices of oral health. A paper survey is provided to the participants both before and after each session, to be filled out and given back to the instructor. The {removed} is seeking a way to input these forms electronically, and have everything saved on a central database. The short-term goal for this web-app is to provide a simple way to submit the forms, and get the data saved online.
