Bonus/Commissions Reports and Entry with Web Interface and SQL Database

We are looking for someone to design and build a database with a web front end. We are going to need to run deposit reports as well as pull specific info from the database and run commission reports from that info. Each Sales person’s information needs to be editable. This includes the commissions percentage for Title Insurance Percentage and Closing Fee Percentage. Commissions fields are Closing Fee Percentage and Title Insurance Percentage. Database fields included at a minimum are Location, Date, GF#, Check Number, Check Amount, Fund Date, Title Insurance, Remained Premium, Closing Fee, HOI Prem, Recording Fee, Courier Fee, e-Recording Fee. Field applied to each Sales Person (This may be renamed): First Name, Last Name, Location, Closing Percentage, Title Insurance Percentage, Is Manager, Manager Percentage. All editable with in the web interface. The web interface should have at a minimum of the following pages: Check Entry Check Modification Sales Person Information (add, delete and edit sales people) Reports (The report selection can be a drop down, check box or links to a specific reports page.) Reports that need to be generated: All sales personnel Sales totals for a given date range. All sales personnel for a given month. Total deposit for a custom date range. Total deposits for a given month. Commission report for a selected sales person for a custom date range. Commission report for a selected sales person for a given month. Commissions report for a selected Manager for a given Month. There are several calculations that need to be made: Determine what the Remaining Premium field, this is a calculation. This is 15% of The Title Insurance amount. Sales Person Commissions: Commission are based on the 85% left from the previous calculation: 85% of Title Insurance x Title Insurance Percentage = Title Insurance Commissions Closing Fee x Closing Percentage = Closing Fee Commissions Closing Fee Commissions + Title Insurance Commissions = Total Commissions Manager Commissions: Managers commissions are based off of 85% of the total Title insurance for a given location that the manager didn’t sell. Calculation: 85% of total Title insurance of a location for a given month x Managers Percentage = Managers commission All reports need to be finished in PDF
