Build a WebsiteWeb Development - w/ Database Look Up Form

I am looking for a Web Developer with experience in creating database "look-up" forms using .kml and .csv files to pull the data from. This would be for a brand new site, and needs to be responsive. Would prefer this to be programmed in a CMS. Here is the gist: -The site would be very simple from a design standpoint, and we would provide the .PSD files. -The site will function as a "Resource Look-Up" in which users in Cook County IL can go to, to find a Coordinator or Provider that is assigned to their township (based on their address). -There will be 4 pages - 1 main landing page and 3 secondary pages -There will be 4 forms: Find Your HDM Provider, Find Your HDM Coordinator, Find Your APS Coordinator, Find Your AC Coordinator. (each work the exact same way) -All forms will only have one field/input - "Enter Your Address". -Once the address is submitted...the database will need to finds the local provider that services the specific township/territory associated with the address that was entered and immediately provide the user with the specified provider’s name, email, phone # as a result. In theory this is how it would work once an address is entered.... 1st - the Database or Web System will need to determine the longitude and latitude of the address entered. 2nd - based on the longitude and latitude determined, the Database or Web System will need to find the township that longitude and latitude is part of (per the .kml file with the township boundary data provided) 3rd - once the township has been determined, the Database or Web System will need to locate the specific HDM provider assigned to that township, per the .csv file provided that maps out each township to the correct provide. Finally - the specific provider that is assigned to the township of the address that was entered should appear as a result to the end user. (See screenshot attached of a basic wireframe) ---There will be 30 townships/territories that need to be covered in the database, all in Cook County, IL. The townships/territories are not based on city/suburb or zip code. (therefore my zip code could be 60619 and yours could also be 60619, however if we entered our full address into this form the resulted provider may be different for each of us). ---We would provide the .kml files to show the boundaries of all 30 townships. This would show (for example)…if an address has 41.716305 latitude and –88.217401 longitude then it falls under the boundaries of Township #1….if an address has 41.677142 latitude and –88.128973 longitude then it falls under the boundaries of Township #3. ---We would provide the .csv files that maps out the providers (and their info) assigned to each township/territory. ---The database will only list one provider for every township/territory. There will never be a reason to show more than one result (provider), as the result will always be exact. Every address has an assigned township/territory, and every township/territory has an assigned provider. (so unlike those retail chain “Store Locators” where you enter your address/zip code and it shows you the 5 nearest Starbucks…with this there will not be a list of the 5 closest providers, only 1).
