Build a WebForm

Hello, I am looking for someone to make a Webform that does the following: It will take users information which include, text, picture, and signature and save them on database to be retrieved and formatted into PDF at a later time. Home page - Instructions Step 1 - First and second name, Last Names Brief instruction for step 2 Step 2 - Introduce ID number Brief instructions on step 3 - Instructions to give camera permissions Step 3 - Take photo Brief instructions on step 4 on signing with their finger Step 4 - Input Signature Step 5 - Verify information and press submit Success page. All steps have next and back buttons Back end must allow me to view how many forms have been collected. View form data Delete forms The forms will need to be printed on paper at the end of the collection time period. Each Letter size page will need to have 5 forms per page. So fpdf or similar must be used to format 5 forms per page and export as pdf. This must be tested in order to be able to export 10,000 forms as a pdf to make sure it doesn't hang or crash. If 10,000 is to big for a pdf then there has to be an option to export 1,000 at a time for example. There is no cms. You can install whatever you want. The form will reside on its own server by itself under a subdomain of the main website. Users on the main website will be sent to fill out the form by pointing them the subdomain. Skills needed: You must be able to use fpdf or something similar App must be able to handle 20000 forms and turn them into pdf You must know how to call camera on webapp You must know how to use signature pad. Webapp must be secured and database must be encrypted
