Code a Small Assessment tool

Award Self Assessment Tool This project needs to be coded in a responsive and mobile friendly manner (HTML 5 and JavaScript) for all client types. This tool will be inserted and or iFramed into a responsive webpage. This tool will also need to be able to function by itself if used in a Kiosk. This tool is to be coded in an manner that does not require any use of a Database but rather uses the session to temporarily save the data. None of the results are to be saved in any manner after the session is completed. Overview Page The over view pages contains 7 different areas of assessment. The areas of assessment each display an 1 to 5 star rating based on the results of the Likert Scale results for each corresponding Assessment Pages. Each of the 7 seven areas of assessment will a link to the corresponding individual assessment page. Assessment Pages There are 7 assessment page that contain instruction and 4 to 6 questions each. Each question will have a set of instruction and a 1 to 5 Likes Scale that is used for scoring Clear All Button should be available to reset the assessment tool Printing Requirements The user should be able too print the following: Print Blank Likert Scales and descriptions PDF Print individual pages and Likert Scale result and descriptions PDF Print Assessment Overview of all Likert Scales results and descriptions PDF Print All - Displays the all Likert Scale results and descriptions PDF Example Page This is a similar assessment tool in functionality, but not same [login to view URL] Note: Code for a L.A.M.P environment (not .NET).
