Single page classic ASP (or PHP) to save and edit data

I need a single page created. Users will access the page by clicking a form on another page. That form will send one form element (an action) to the page. The page will then create an initial entry (EvaluationFormSummaryID) into a database and read the details of the rest of the page from the database. The page will be a form that users fill out. The page needs to be dynamic - the number of questions in the form will come from the database and can expend / contract, depending on the form requested from the database. The page needs to draw the form per the structure from the database and then allow data entry and save. On save, the form will post to itself and the data will be saved in the database. The structure of the data in the form: New records are created in the summary table (tblEvaluationFormResultsSummary) - the rest of the structure of the form is: Form overview is stored in tblEvaluationForms Form structure is broken into Sections (tblEvaluationFormSections) Each section has one or more questions (tblEvaluationFormQuestions) Each question has one or more possible outcomes (rating options) (tblEvaluationFormQuestionRatingOptions) The results are stored by summary (tblEvaluationFormResultsSummary) then by details (question by question) (tblEvaluationFormResultsDetails) The page will also need to allow editing of existing results data with data pulled from the database, when a form with the relevant EvaluationFormResultsID is posted from a different page. Layout can be based in a table or in DIVs for layout. I am supplying current CSS files and a sample of the page for reference. Database tables (SQL server) are included as an example. All code (preferrably classic ASP / VBScript but can be PHP) must be clearly commented and a link to a live URL working example allowing the creation and editing of data to be provided.
