A couple of questions: 1. will the registered user have always one form and only ? 2. do you need to populate the form's data with any default values? 3. I will create a shortcut for the form so that you can import it in any WP Page. Is that sufficient ? 4. do you need any other admin page such as listing the forms or viewing a form of a specific user ? 5. Will I use the template of the site? 6. Can you describe a little the functionality of the plus '+' sign ? Does this mean that in that section it will have multiple rows ? 7. Can you provide all the values of the drop downs ?
