Custom Quote in Salesforce

Likely needing to use Apex or Visualforce, we are in need of a custom quote that will have the ability to achieve the following:
- Follow the same design & layout that we have predefined (sample attached)
- User needs to be able to type a paragraph or more of detail on each quote line and be able to visualize several lines, if not all at one time.
- Users would like to select all items and then be able to make large amounts of notes next to each item
- Need ability to modify quote which would update the revision number on the quote and save copies
- Needs to be able to choose from two quote layouts, quote layout A to have random generated quote #, Quote layout B needs to follow convention YYMMM### (### denotes number of quotes created in calendar year)
- there is an existing customization which has a link to a SharePoint folder, ideally quote object would have that link (not a deal breaker)
