Create a simple new UI of an existing software

We have a software that has a simple main form and we need this form to be totally re designed but keeping its main functionality. Your job is to create a this new form, totally different from he existing one and deliver the new graphics so we can compile this new application again. This is a simple task for a good designer, all we want is that out customer don't think that his software is exactly the same of another customer, so so whatever you want, just make it look different.
