Website header graphic, link bar/buttons

Need a website header graphic made. Finished file will be editable (.psd?) Finished text on header would read:
1st line of text:
"SAHotShot"
2nd line of text:
"Your general store delivered to your door."
Needs to have a link bar, home, contact us, etc.
Need 2 images made, "100% Satisfaction guaranteed" seal.
Second image would read, "Only $4.95 for delivery! FREE on orders of $40 or more."
I'm attaching my websites logo, its the image with the car. This would be in the upper left corner of the header graphic.
I'm attaching a very rough idea of what the graphic should look like. Please use colors from the car logo, or colors that work with the colors contained in the car logo.
Please do not use the colors in the header graphic I've supplied, thats just to be an example of a header graphic should look like.
