Boostrap theme customisation

We require someone with significant BOOTSTRAP experience to help in customising a theme similar to:

The coder will receive an excel spreadsheet with a list of form elements, names, validation requests etc. and be required to create a view based on there. There is no PHP/Javascripting required. Each sheet in the file corresponds to one HTML page. We will provide the framework into which you can add the relevant forms.
In total you need to create ±20 pages.
Before you can be considered for this job you will be required to complete a test on interviewstreet. As this is a long term position, we want to ensure that you are able handle the task well. If you do not have the relevant experience , please do not waste your time.
Milestones will be created for the work and only paid once we are 100% satisfied with the work.
Additional work will be awarded if you provide quality work.
Communications skills are a priority. You must be able to provide feedback once per 12 hour period. You will be tested on this from the moment you respond to this posting.
The user will be given the template files to work with. You will NOT be starting from scratch. These files remain the property of Phoenix LTD and are to be deleted once the project is finished.
The coder is expected to add value to the process and make suggestions on how to better display data.
The time frame is 3 days. There is a USD20 bonus for early completion
