PDF Column to Excel Column Conversion

I have a large list that I need to create an excel sheet.
The problem is the list is in PDF format and doesn't convert well to text and I can't seem to get excel to format the list correctly into columns.
I need someone who can figure out the conversion and get each column from the PDF to match column in the Excel sheet.
The only change I need is to also separate the name field into 3 columns of first, last, and middle names.
The PDF is attached.
