Modify and complete formatting of a form in Microsoft Word

I have a form that I have worked on that is almost complete. I need someone to do the finishing touches. You MUST have experience with form creation in Word, i.e. use of Content Controls, etc. As it stands now, the form is complete with its content, but I need things like the content control boxes and form text to be fixed (i.e. when you type into the content boxes, the text that begins filling them does not cause the rest of the document to shift down). This should be a very easy project for someone with experience with creating Word forms, since, as you'll see, the entire thing is created and and I just need help with formatting details.
