CSS - Need a clean form and css created from merging of two page styles.

Looking for a clean form created that perfectly duplicates the graphics and menuing system and aspect ratio/ margins and header and footer on this page: [login to view URL] and then creates a nice clean white background form (replace the green form entirely) using the form fields above but using the CSS and styling from this page: [login to view URL] The [login to view URL] page is a wordpress template and is full of garbage when downloaded and I while I have permission to work on both systems I don't have access to the wordpress template and will host this on a different server without wordpress. It needs to be pure HTML 5 and CSS and eliminate as much of the wordpress stuff as possible doesn't need to post anywhere... I'll handle that part. Also the page needs to be albe ti execute jquery ajax calls so the most recent jquery library should be included...
