Looking for a Form that export ADF/XML format

Hi there, I can create a nice and responsive form which when submitted will generate an ADF/XML file on the server. Front end would be HTML/CSS (Bootstrap 4.5), the backend would be PHP. Ready to start as soon as awarded, and should need no more than two days to complete the work, depending on the number of fields and the complexity of the form. The code would be clean and commented.
