Build a Video Submission website

Here's what we need:
A website where users can submit a video to us for our professionals to give them feedback and tips.
Front end
Home page users will be able to setup an account/login to their account and give the following info
Name:
Address:
Town:
State:
Phone:
Email:
Once users have registered and logged in they will be taken to a page they can add/edit/view upload their OWN video submissions. On each video they will see/add the comments posted by our admins and will be able to reply to them.
ADMIN Section
Admins can view all video submissions sorted by Date/rating/favourited
Admins will be able to Rate videos out of 5 stars
Admins will be able to edit/add/remove comments on each video they view.

Feel free to use a pre-made site/script we want this to be a very simple site that is very user friendly and works very easily.
Freelancer will submit 3 designs before programming begins. And will setup website on our hosting platform once completed.
Site will be very simple and MUST be mobile friendly so users can submit videos from their phones.
Budget is $300 CAD and we need this done in one week.
