Add Back end to site to add items to inventory

I need the site [login to view URL] to be modified so there is a back end where I can securely login, where there is a very simple upload form for me. I need it to have 5 or 6 fields in order to include all the information found for a product. This includes
Title
Subtitle
Description
Price
Category (Stainless Steel Tank, Fiberglass Tanks, Steel Tanks)
ID #
and an image
So,
1. you would need to create a database (or I could do it for you and provide you with the info), then create a record insertion form with the ability to insert an image.
2. A front end page that links to the appropriate category and lists all the items in that category, preferably with a thumbnail image
3. The description page itself, that displays the info.
4. All URL's should be Search engine friendly.
I have set up a few sample products on the site for you to use as a template on the page so you should be able to see how I want it..
