I have a PDF that I would like someone to convert into a fillable PDF, as well as make some small modifications to the file.
1. If someone checks the box to allow an email address, I'd like the 'requested email address' to be automatically filled out with this format: first letter of the first name, lastname, @lyncorp.ca. For example, if someone fills out the form, and they put in John Doe as the new user, then check the email box, the requested email address should automatically be created as jdoe@lyncorp.ca. The user can then edit that email address, but it would be auto-populated for them.
2. Please put a vertical line between the two columns for network folders and JobBOSS folders.
3. Do not allow the network folders nor the JobBOSS groups checkboxes to work unless someone first checks the first check box in that column. So, you can't select network folders unless you FIRST check the box that says 'Network Access?'
4. I'd like all this to fit on a singe page.
