Need a coder with above listed experience to implement a few Yes/No pre-order screening script like this one:

Mine questionnaire is much simpler than the above example.
1. I will provide a list of 3-4 Yes/No questions and response;
2. You need to implement it into our current website and it will include creation of 2-3 contact form/upload sort of html page.
3. It is extremely important that the code you deliver to be absolutely CLEAN, no mess and no unnecessary includes, the naming of the JS code should make sense, it's not OK to use asis from the example site.
4. No graphics of what-so-ever is needed, but some CSS work to style the texts and buttons needed to be done to my satisfaction.
It's a small project, do not bid more than $55.
