295983 Form Data

I have 2 html files. I would like the data inputted to go to a database and and then updated on a results html file. There are two inputs that can vary from time to time and I need the ability to insert those as they change.

You will understand (I hope) when you see the html file.

The other file ([login to view URL]) is a Membership Detail file that I need to have emailed back to me in a structured manner.
