On-Line form to E-Mail

Hello, need a on-line form that can automatically e-mail the completed form. I have attached a copy of the form and the fields required. He would also like to accept credit card info so if it can be made secure would be a real plus. The attached form is just a copy but I would expect it to be designed to match the existing web site in color and format. I can and will provide logos.
