Make an existing email signup form redirect to a thank you page and trigger a tracking code

Situation is like this:
- I have an existing website that has a Mailchimp signup form on it.
- I want to track email signups with a tracking code that has to be placed on a "thank you" page that people are redirected to after entering their name and email details.
- This redirection doesnt happen currently so I cant place the tracking code.
The task is:
1. to redirect the page to a "thank you" once people entered their email address and click "submit" and also place the tracking code on the thank you page
2. to make the email address be passed through via the tracking code (there is a placeholder in the tracking code that needs to pass through the email address that people entered into the form).

Actually with mailchimp we have option to redirect to Thank you page but there isnt option to add the email address in the url so may be can i see your tracking cos so I can see if I can do somethign over it please?
