WordPress - Unable to Receive an email with a form + develop a new page (new form)

I need a freelancer who can:
1- Fix a problem on a WordPress website: The person who visits the site must complete a form to enter their email and information concerning them in order to download [login to view URL] form is already developed but the problem is I do not receive an email containing the information entered (and I do not see in WordPress the list of people who filled out the form).
2- Add a new page (form) with the following information :
- Description
- First Name / Last Name
- Phone
- Function.
The aim of this page to get the info about the subscribers into an event (Webinar).
The page should have the link below:
