Migration of MySQL database from my old website to new website with some modifications

I want to migrate one MySQL database from my old website to new website with some modifications.
Modifications include:
1) import of old database entries in to new database (both to be done online)
2) some modifications into the new MySQL db(adding or removing or editing fields)
3) making the new website to work.
The website pages are already created.
The work is only related to MySQL databases and in to some php files in order to make the database fields compatible with new website.
Knowledge of PHP and MySQL is mandatory.
Project is urgent need to be completed within 7 days.
Basically I have one old matrimony site and one newly purchased matrimony script.
I want the data of members from old matrimony site to be added to db of newly implemented website.
The url of registration form (db fields) old website is http://bhoimangalvivah.com/registration_single_page.php
and url of new website user registration form is http://bhoimangalvivah.com/new/register.php
Also check attached image of related fields.
The image at the center is a registration form for old website which is one page.
and the three images around it are new website registration form which is of three pages.
I have shown by arrow which old field is to be added in which new field.
