Add Upload column to table

This is jQuery table based on SQL server
1. Add 3 columns each column will have upload+image
2. On upload the image need to go to folder named like the name of the facebookID column
3. File names
a. [login to view URL]
b. [login to view URL]
c. [login to view URL]
