NEED ASAP!

The files screenshot shown in the attached image are ready to be configured to work with a wordpress page (Divi Theme). I've already completed some of the integration but need help with the rest. You can see current version (link below) and an attached image of what I've started (page_view_in_wordpress). You will notice that the formatting is off but it's because I don't know how to integrate the javascript properly.

I need help finishing the page AND adding both the "download results" buttons with email functionality and the "more details" button with dropdown functionality.

I have all the files for the design of the (current) page:
