embeddable field where objects can be dragged and dropped

I need a simple, embeddable web app where users can choose from a very limited set of objects, that I provide, and drag/drop/arrange them on a small "canvas" and send the design to me with their name and email.

The draggable/droppable objects should probably be div elements so that when I want to add or delete available objects for use, I can just go into the source code and add or delete divs.

This "design app" should be called when users click on a button- "design your own shirt" on a page within my current website
