Create WEB Page FORM in html / php

Require a web form completed in html / php. This form will be used to add/update/delete records in a mySQL database. Also required on the form is a table of the existing records for the user to choose a record for editing or deleting.
Also required is the capability to upload files to the server. Files to upload are .jpg , .mp3 and .mp4. Some of these .mp4 files are large and can be up to 50 MB.
The database setup is complete and the php scripts functioning to add / delete and update the database are functioning. (from an iOS device) This web form needs to call theses existing php scripts with the correct parameters. There is no file upload capability in the existing scripts. Existing php scripts include:
retrieval of records ( JSON )
update of a record
insert a new record
delete a record (with deletion of associated files to this record)
There are about 15 fields.
One of the fields needs a pick list with about 8 items
One field is a text field that would need to scroll which would contain up to a couple pages of text.
The form will not be used by the general public, just used by one administrator to maintain the database and associated files and thus, just needs to reside on a local computer.
