Fix HTML Web Form on Website

I need someone that can add some checkboxes and make some fields "required" on my form that currently works. I also need someone to make my radio boxes look a bit better as it currently looks a bit strange and the text is a bit small.

Someone with HTML skills and a bit of design/ User Experience skill would be great.

You would be styling the form using Infusionsoft web form code that I can provide you.
