Covert and Enhance .ASP website into PHP +AJAX

Take an existing online web appointment system and enhance it and add a few more modules.
It is currently ASP. It needs to be changed to a more interactive system. We are looking at PHP +AJAX and web design done in Dreamweaver:

(The enhancements to add are the areas Set Up & Maintenance: Administrator and the Reports sections are the ones we are looking to have into our existing application)
If picked you agree to the following terms:
1) Weekly feedback and link to show development and progress
2) Hosting on your server for testing
3) Ability to make changes and enhancements that we feel
4) Support for at least 30 days after project is completed
5) Be professional
6) Do not use any bad codes or programming to time out project
7) Be clean with programming and detailed with comments within code and within the code (this is a must and will be verified before final payment is released)
We will be hosting this site on Godaddy (Linux server).... we will not give out our hosting information. You must be able to communicate any set up instructions for us to make it work on our end.
I will supply to the provider I pick, all the original files.

One note is that on the flashappointments site there is a payment credit card section, we will still be using a https certificate for this, but there will be no payment generation on our system – customers will enter their payment information and the system will just store it for the admin to manually process the payment using another program like Quickbooks.
PAYMENT INFORMATION:
We will post the winning bid in 2 payments: the 1st 50% for when the application is shown on your server for testing. The 2nd 50% will be relased when the application working files are delivered to us and tested on our server.
We will only use the GAF escrow function for payment. Once the project is complete and we receive all orginal files, codes, etc. we will then release the second 50% amount to you.
